Heart Failure Outpatient Management
Developed in collaboration with leading cardiology thought leaders, HeartAware is a software-based remote patient monitoring system. Our experience through pilot programs quickly revealed heart failure patients already had the appropriate tools at home to self-monitor but needed further guidance and confidence to stay engaged in utilizing them. HeartAware not only transmits and triages patients to the healthcare team, but also engages the patient to recognize their own symptom changes through a color-code system and pop-up alerts. Thus, providing them the opportunity to connect to their healthcare team. The collaborative relationship with healthcare providers proved that we did not need another solution that data dumped on healthcare systems, but rather created a two-way partnership between patient and healthcare team.

In addition to reporting key markers, HeartAware allows the patient to report any other concern they may be feeling at the moment. It’s a convenient way for them to connect with their physician or nurse and report issues of concern.

Report General Condition
Patients use this simple feature to report how they're feeling at the moment, from feeling fine, to dizziness, to shortness of breath and other types of feelings.

View Patient Report Log
In the “My Reports” section of the mobile app, patients can view the ongoing log of their reports, including those other issues that may concern them. It gives them an instant view into the same key markers their physician and nurse are seeing each day.

Messaging/Chat
Patients can view messages from their physician or nurse and engage in real-time chat when the medical professional wants to connect with them. It gives medical professionals an instant connection to their patients when needed and improves overall communication between patients and their physicians and nurses.

Emergency Call Button
The HeartAware mobile app can be customized with a 911 call button, or it can be programmed to call physicians' offices.

Provide Patient Reward Statements
When patients report for consecutive days over consecutive 7-day periods, they will receive rewards showing their consistency in reporting. This also applies to successive days remaining in the "green" (good) zone.
Behavior/Lifestyle Resources
Heart Failure patients may access up-to-date information on diet, exercise and other behavioral and lifestyle information that promotes healthy living. All coming from recognized, trusted sources. This is content that will change monthly.

My Health Journal
Provides an area for heart failure patients to enter relevant information about their heart failure that can be seen by patients in their My Reports section along with their healthcare providers in the dashboard.
