• Remote Patient Monitoring Without Bulky Costly Hardware

  • Keep Patients Healthier

  • Reduce Unplanned Hospitalizations

  • Intervene Before Critical Event

30-day readmissions for heart failure patients are still a continued battle with little success in reducing hospitalization rates. Readmissions consume resources and remain a substantial burden on the US healthcare system. HeartAware is a software-based remote patient monitoring system that not only keeps health systems connected for real-time health metrics but has been designed to engage patients in recognizing their own fluctuations and acting on them.

Keep Patients Healthier
By providing physicians and nurses with daily key markers of weight, blood pressure and pulse, they can monitor patients’ conditions and spot trends that lead to heart failure and eventual hospitalization.
Reduce Unplanned Hospitalizations
By simply reducing one unnecessary hospitalization in a year, the savings more than justifies the small annual cost of HeartAware. Plus, patients are able to stay home and live a healthier life.
Intervene Before Critical Event
When key markers increase, they indicate that heart failure patients are likely trending to a critical event that usually requires hospitalization. The key markers give physicians and nurses the information they need to intervene and help patients return to more stable, healthy life.
Monitor Health Markers
The key health markers for congestive heart failure patients include weight, blood pressure and pulse. HeartAware provides medical professionals with the real-time view of patients’ conditions so they can intervene and take action as needed.
A Software-Only Solution
Using a mobile app and online dashboard, HeartAware is far more affordable than solutions that require hardware; and can be applied to a broader group of heart failure patients.
Impact Behavior & Lifestyle Changes
In the mobile app, heart failure patients may access content that is relevant to their ongoing health. This includes access to up-to-date information on diet, exercise and other behavorial and lifestyle information that promotes healthy living. These come from recognized, trusted sources and will change monthly.
Not a Data-Dump Solution
Managing a patient’s heart failure should be a partnership between a patient and their healthcare team. HeartAware takes the unique approach of focusing on bridging that partnership through strong patient engagement features. These features guide patients in self-managing while providing coordination with their healthcare team through the dashboard.

How It Works

HeartAware is a patent-pending software platform that manages key CHF patient marker information in real time.

Submit Health Markers
Patients submit Weight, BP, and Pulse, along with an indication how they feel today. They may also view their daily report log and communicate w/ Physician/Nurse via mobile app.
Track Health Markers
Physicians and nurses use the online dashboard to manage all aspects of patient reporting, tracking their key markers and being alerted to movement between levels.
Be Reminded
Patients may elect to be reminded to submit their key markers daily, along with reminders to take medications, with notifications to the mobile app.
Personalize
The HeartAware Dashboard enables physicians and nurses to personalize patient baseline, warning and critical levels, establish chat sessions, send alerts, and view patients' history logs.

Patients Benefit Using Remote Monitoring

According to our initial pilot survey, heart failure patients reported an overall improvement in their ability to communicate with their healthcare provider and grew their confidence in self-managing their heart failure.

Improving and Innovating

Heart Failure Outpatient Management

Developed in collaboration with leading cardiology thought leaders, HeartAware is a software-based remote patient monitoring system. Our experience through pilot programs quickly revealed heart failure patients already had the appropriate tools at home to self-monitor but needed further guidance and confidence to stay engaged in utilizing them. HeartAware not only transmits and triages patients to the healthcare team, but also engages the patient to recognize their own symptom changes through a color-code system and pop-up alerts. Thus, providing them the opportunity to connect to their healthcare team. The collaborative relationship with healthcare providers proved that we did not need another solution that data dumped on healthcare systems, but rather created a two-way partnership between patient and healthcare team.

Report Key Markers

Report Other Concerns

In addition to reporting key markers, HeartAware allows the patient to report any other concern they may be feeling at the moment. It’s a convenient way for them to connect with their physician or nurse and report issues of concern.

Report General Condition

Patients use this simple feature to report how they're feeling at the moment, from feeling fine, to dizziness, to shortness of breath and other types of feelings.

View Patient Report Log

In the “My Reports” section of the mobile app, patients can view the ongoing log of their reports, including those other issues that may concern them. It gives them an instant view into the same key markers their physician and nurse are seeing each day.

Messaging/Chat

Patients can view messages from their physician or nurse and engage in real-time chat when the medical professional wants to connect with them. It gives medical professionals an instant connection to their patients when needed and improves overall communication between patients and their physicians and nurses.

Emergency Call Button

The HeartAware mobile app can be customized with a 911 call button, or it can be programmed to call physicians' offices.

Provide Patient Reward Statements

When patients report for consecutive days over consecutive 7-day periods, they will receive rewards showing their consistency in reporting. This also applies to successive days remaining in the "green" (good) zone.

Behavior/Lifestyle Resources

Heart Failure patients may access up-to-date information on diet, exercise and other behavioral and lifestyle information that promotes healthy living. All coming from recognized, trusted sources. This is content that will change monthly.

My Health Journal

Provides an area for heart failure patients to enter relevant information about their heart failure that can be seen by patients in their My Reports section along with their healthcare providers in the dashboard.
